Thanks for taking the time to fill out our Genealogy Request Form. We will attempt to answer all your questions, provide death record information, burial location of deceased within the cemetery as well as a location map. Unfortunately not all data desired is available. Therefore, results may reveal all information sought, some of it, or in some instances, no information at all. We do not have copies of birth or death certificates within our files.
NOTE: As a general rule, we cannot provide the names of parents, children, or other relatives because family relationships are not recorded. If a person is buried within a family lot, we can tell you who is buried on the lot with them.
Patron Information and Research Fees: Mount Calvary Cemetery Group will provide your requested information for a maximum of five (5) seperate individuals for a fee of $40.00. Requests for additional names/individuals beyond five (5) will incur a $5.00 per name fee. Please make checks payable to the Mount Calvary Cemetery Group or payment can be made via a credit card.
All searches are made from Mount Calvary Cemetery Group records only and do not include such sources as death notices, obituaries, or death certificate information. All genealogy requests are answered by mail only. They are completed in the order in which they are received and will be mailed to you within four-to-six weeks from date of receipt. If our search turns up empty-handed, we will notify you of this via email.
